CDS has an automated appointment reminder that will phone
you two days before your scheduled appointment to confirm
your visit. When you arrive for your appointment, you
will be asked to complete a patient information form.
This form records current medications and health problems.
CDS will refer to this form to contact you after your
appointment, so please correctly record your current address,
phone number and insurance. You will also be asked to
complete a HIPAA acknowledgement form.
When your visit for the day is complete, the front office
will review the doctor's orders with you and schedule
any lab tests, procedures, or follow up visits. Payment
is expected on the day of your appointment, so please
have your insurance information on hand.
